Name Date Dear Sir/Madam: I have ____ (explain your problem here: ex, high cholesterol, hypertension, CHD, low HDL, high triglycerides, IBD, diabetes, etc. Review this diagnosis with your doctor). My doctor and I have tried different treatments without adequate success. At the request of my doctor, I had my blood analyzed for fatty acid composition. The purpose was to identify abnormalities and deficiencies of essential fats (EFs), so that my doctor could prescribe a treatment to correct them. As you know, if abnormalities and deficiencies of EFs are not corrected, the result would be deterioration of my organs and drastic impairment of my health. My health care costs would be dramatically increased in the long term. The diagnosis of abnormalities of EFs is highly complex and has been written about in over 500 medical journals. Treatment of such abnormalities has no known danger and is likely to correct EF deficiencies. After discussing these issues with my physician, it was his opinion that I should have a blood test for fatty acid analysis. Any abnormalities found could then be corrected. I do not want to develop a severe health problem because I had an easily correctable nutritional abnormality that was not properly diagnosed. The test was done and the bill was submitted for reimbursement. According to my research (OR SAY "according to Dr. Siguel, an expert in this field"), insurance companies and Medicare commonly reimburse for this type of test in patients like myself.
